If I understand your question, you are asking can patients with SCHAD deficiency have high and low blood sugar levels. The answer is yes. Anytime you eat, your sugar increases about the normal upper limit, but starts to drop immediately as the body releases insulin to start cells metabolizing the sugar. Patients with SCHAD deficiency are usually overly sensitive to insulin and so in contrast to others, whose activity drops off as glucose levels decrease, patients with SCHAD deficiency overshoot and end up with sugar levels that too low. Your metabolic doctor should be able to help you develop a diet that minimizes the swing in glucose level and most importantly prevents you from bottoming out and getting hypoglycemic. Hope this helps. Dr. Vockley
